Knee Pain After 40: Physiotherapy Solutions That Work

Knee pain after 40 is common, but it is not something you simply live with. This guide covers why it starts, the exercises that help most, and when to get assessed.

Why knees start complaining after 40

From our late thirties, the cartilage that cushions the knee gradually loses water content and the quadriceps muscles that protect the joint quietly weaken, especially in desk-bound years. Add Indian daily life, squatting for pooja, sitting cross-legged for meals, two-wheeler kicks and stairs without lifts, and the knee works harder than most joints in the body. Weight matters too: every extra kilogram adds roughly four kilograms of load to the knee with each step downstairs.

Five exercises worth doing from today

Do these on alternate days to start, building to daily. Mild muscle soreness is fine; sharp joint pain or swelling the next morning means step back a level.

1. Quads isometrics

Sit with the leg straight, press the back of the knee down into a rolled towel, hold ten seconds, ten repetitions. This is the safest way to wake up the knee's main protector.

2. Straight leg raise

Lying down, lock the knee straight and lift the whole leg to 45 degrees, hold five seconds, ten repetitions per side.

3. Sit-to-stand

Stand up from a firm chair without using your hands, sit back slowly over three seconds. Start with eight, build to fifteen. Slower is stronger.

4. Step-ups

Use the bottom stair. Step up leading with the painful leg, step down slowly. Ten each side. Hold the railing if balance is uncertain.

5. Hamstring and calf stretches

Thirty seconds each, twice per leg. Tight muscles behind the leg quietly increase pressure on the front of the knee.

Book an assessment if pain has lasted more than two or three weeks despite sensible self-care, wakes you at night, or the knee locks, gives way or swells repeatedly.
