Red flags: see a doctor first

Get medical attention promptly if you have: an inability to put weight on the leg after an injury, obvious deformity, a hot swollen knee with fever (possible joint infection, which is an emergency), a knee locked and unable to straighten, calf swelling with redness or warmth, or night pain with unexplained weight loss. Everything else is usually safe to begin with physiotherapy assessment.

Four safe starter exercises

  1. Static quads: sit with the leg straight, press the back of the knee down into the bed by tightening the thigh, hold for 5 seconds, 10 repetitions. Safe for almost every knee.
  2. Straight leg raise: lying on your back, keep one knee bent, lift the other leg straight to about 45 degrees, hold for 3 seconds, lower slowly. 10 repetitions.
  3. Sit-to-stand: from a firm chair, stand up and sit down slowly without using your hands, 8 to 10 repetitions. Use a higher chair if this is too hard at first.
  4. Hamstring stretch: sitting on the edge of a bed with one leg out straight, lean gently forward from the hips until you feel a stretch behind the thigh, hold for 20 seconds, twice per leg.

How long does recovery take?

It depends heavily on the cause. Simple overload problems often settle in 4 to 6 weeks of load management and strengthening. Meniscus and ligament irritations typically need 6 to 12 weeks. Osteoarthritis responds over 8 to 12 weeks of strengthening and then rewards ongoing maintenance. Post-injury and post-surgical timelines run longer and are guided by testing rather than the calendar. Frequently Asked Questions

Do I need an X-ray or MRI for knee pain?

Often not at the start. Clinical examination identifies most knee problems reliably, and scans frequently show incidental changes that were never the cause of pain. We refer you for imaging when the examination suggests it will change the treatment, such as after significant trauma or when surgery is on the table.

Is climbing stairs bad for my knees?

No, though a painful knee may need stairs dosed carefully for a while. Stairs load the kneecap joint more than level walking, so during rehab we may modify how much you climb, then rebuild it as strength returns. Avoiding stairs forever weakens the knee further.

My knees make cracking sounds. Is that harmful?

Painless clicking and cracking is common and almost always harmless; joints are simply noisy sometimes. It matters only when the sounds come with pain, swelling or giving way, in which case an assessment is worthwhile.

Should I stop walking until the pain goes?

Usually the opposite. Comfortable walking keeps the joint nourished and the muscles active. What we adjust is the dose: shorter, more frequent walks on even ground during a flare, building back up as the knee settles.

Do knee caps and braces help?

A simple sleeve can give comfort and confidence in the short term, and specific braces have a role in certain ligament injuries. None of them strengthen the knee, so we treat braces as a bridge while the muscles are rebuilt, not a destination.

When should I see a surgeon instead of a physiotherapist?

After major trauma with deformity or inability to bear weight, for a locked knee, or when a proper course of rehabilitation has failed and imaging shows a clearly surgical problem.
