Patients in Ahmedabad often ask: "Should I see a physiotherapist or a chiropractor for my back pain?" It's a fair question, both professions treat pain without surgery or medication, and their approaches overlap. But they differ significantly in philosophy, training, scope, and the evidence behind them.
As a physiotherapist with over 10 years of clinical experience, I've treated thousands of patients in Ahmedabad. This guide explains both disciplines honestly, so you can make an informed choice.
What Is Physiotherapy?
Physiotherapy (also called physical therapy) is a regulated, evidence-based healthcare profession focused on restoring movement, strength, and function after injury, surgery, or illness. Physiotherapists assess the whole body, identify the root cause of pain or dysfunction, and create a personalised treatment plan.
What Physiotherapists Do
- Manual therapy, joint mobilisation and soft tissue techniques (Maitland, Mulligan)
- Therapeutic exercise, progressive strengthening, stretching, neuromuscular re-education
- Electrotherapy, ultrasound, TENS, IFT for pain relief and healing
- Post-surgical rehabilitation, structured recovery after knee replacement, ACL surgery, etc.
- Sports injury management, from acute sprains to return-to-sport protocols
- Neurological rehabilitation, stroke recovery, Parkinson's, nerve injuries
- Women's health, pelvic floor, postnatal rehabilitation
Qualifications in India
Physiotherapists in India complete a 4.5-year Bachelor of Physiotherapy (BPT) degree, including a 6-month internship. Many go on to complete a 2-year Master of Physiotherapy (MPT) with a specialisation, in musculoskeletal, sports, neurology, or orthopaedic physiotherapy. The profession is regulated by the Indian Association of Physiotherapists (IAP) and is recognised as an Allied Health Profession.
What Is Chiropractic Care?
Chiropractic is a healthcare discipline focused primarily on the diagnosis and manual treatment of musculoskeletal disorders, especially the spine. Chiropractors use spinal manipulation (also called adjustments) to align the vertebrae and relieve pressure on nerves.
What Chiropractors Do
- Spinal manipulation/adjustment, high-velocity, low-amplitude thrusts to mobilise joints
- Soft tissue therapy, massage and trigger point release
- Posture analysis and correction
- Lifestyle advice, ergonomics, exercise, nutrition
Qualifications and Regulation in India
Chiropractic is not regulated by any central law in India. There is no statutory body equivalent to the physiotherapy councils in most states. Chiropractors may hold a 5-year degree from international institutions (USA, UK, Australia) or shorter domestic diplomas. This makes the quality of training highly variable. Always verify credentials carefully before visiting a chiropractor in India.
โ ๏ธ In India, the term "chiropractor" is not legally protected. Ensure any practitioner you consult holds an internationally recognised chiropractic degree (DC or equivalent) from an accredited institution.
Side-by-Side Comparison
| Feature | Physiotherapy | Chiropractic |
|---|---|---|
| Primary Focus | Full-body movement, function, strength | Spine alignment, nerve pressure |
| Main Techniques | Exercise, manual therapy, electrotherapy | Spinal manipulation/adjustment |
| Scope of Practice | Broad, orthopaedic, neuro, sports, women's health, post-surgical | Mainly musculoskeletal, emphasis on back/neck |
| Evidence Base | Extensive, thousands of RCTs across all conditions | Moderate for back/neck pain, limited for other claims |
| India Regulation | Regulated, IAP, state-level councils | Not centrally regulated in India |
| Post-Surgery Care | Yes, core part of practice | Rarely involved |
| Neurological Rehab | Yes, stroke, Parkinson's, nerve injuries | No |
| Treatment Duration | Active rehab, empowers patient independence | Often requires repeated ongoing visits |
| Long-term Goal | Patients learn to self-manage; fewer visits over time | Maintenance adjustments often indefinite |
When to Choose Physiotherapy
Physiotherapy is the better choice for the vast majority of musculoskeletal and pain conditions. Choose physiotherapy when you have:
- Back pain, neck pain, or sciatica
- A sports injury (sprain, strain, muscle tear, ACL)
- Post-surgical rehabilitation (knee replacement, hip surgery, rotator cuff repair)
- Joint pain from arthritis or wear-and-tear
- Frozen shoulder or shoulder impingement
- A neurological condition (stroke, Parkinson's, neuropathy)
- Chronic pain that has not responded to other treatment
- A desire to improve strength, posture, and prevent future injury
โ In 10+ years of practice, physiotherapy has helped 2,000+ patients in Ahmedabad recover from conditions ranging from disc herniations to post-surgical rehab, without surgery, and with lasting results.
When Chiropractic Might Help
Chiropractic care has moderate evidence for short-term relief of acute low back pain and neck pain. It may be appropriate when:
- You have acute, uncomplicated mechanical low back pain
- Manual spinal adjustment has helped you in the past
- You are seeing a qualified, internationally-trained chiropractor
However, chiropractic is not recommended after spinal surgery, for patients with osteoporosis or bone fragility, inflammatory conditions like rheumatoid arthritis, or any neurological conditions. High-velocity neck manipulation carries rare but real risks.
The Indian Context: Availability and Regulation
In Ahmedabad and across India, physiotherapy clinics are widely available, regulated, and affordable. Physiotherapists at reputed clinics hold BPT or MPT degrees, and are easy to verify.
Chiropractic care in India is less widespread. Practitioners vary significantly in training, from internationally qualified DCs to those holding short domestic diplomas. The lack of central regulation makes consumer protection harder. Before visiting a chiropractor, ask to see their DC degree from an accredited institution.
At PhysioBuddy, we treat back and neck pain every day using evidence-based physiotherapy, Maitland and Mulligan manual techniques combined with therapeutic exercise. Most patients see significant improvement within 8โ15 sessions.
Our Verdict
For most conditions, including back pain, neck pain, joint pain, sports injuries, and post-surgical rehab, physiotherapy is the evidence-based first choice. It is broader in scope, better regulated in India, and its goal is to make you independent, not reliant on repeated visits.
Chiropractic care can offer short-term relief for acute spinal pain if performed by a qualified practitioner, but it is not a substitute for comprehensive rehabilitation.
If you are unsure, start with a physiotherapy assessment. Your physiotherapist can evaluate your condition and, if appropriate, refer you on. The reverse, a chiropractor referring for physiotherapy, is far more common.
