Tennis Elbow Treatment: Physiotherapy Exercises & Recovery Timeline

Tennis elbow (lateral epicondylitis) is the most common elbow condition, yet most people try to manage it with rest and painkillers alone. Physiotherapy produces dramatically better long-term outcomes.

What Is Tennis Elbow?

Tennis elbow (lateral epicondylitis) is degeneration and microtearing of the extensor tendons at their attachment to the lateral epicondyle, the bony bump on the outside of the elbow. Despite the name, only 5% of cases occur in tennis players. It is most common in people aged 35–55 who perform repetitive wrist extension activities, typing, painting, carpentry, cooking, or any repetitive grip work.

💡 Modern research shows tennis elbow is a tendinopathy (degenerative tendon change), not tendinitis (active inflammation). This changes treatment significantly, anti-inflammatory approaches alone are insufficient.

The Most Effective Tennis Elbow Exercises

1. Tyler Twist (Eccentric Wrist Flexion)

Hold a Therabar or flexible rod in both hands. With the affected arm fully extended, rotate the bar by flexing the good wrist while the affected wrist fights the motion eccentrically. 3 sets × 15 reps. The most evidence-based exercise for lateral epicondylitis.

2. Wrist Extensor Eccentric Strengthening

Rest forearm on a table, hand hanging over edge holding a light dumbbell (0.5–1kg). Use the good hand to raise the weight into wrist extension. Then lower it slowly (3–5 seconds) using only the affected hand. 3 sets × 15 reps.

3. Ball Squeezing

Squeeze a soft ball or putty for 5 seconds, release. 10–15 reps. Improves grip strength and stimulates tendon remodelling.

Manual Therapy for Tennis Elbow

Dr. Maitri Kothari uses Mulligan's technique for lateral epicondylitis, a lateral glide of the radial head during wrist extension, which consistently produces immediate pain reduction. Deep transverse friction massage to the tendon insertion stimulates collagen remodelling. Joint mobilisation of the elbow and wrist addresses contributing factors.

Frequently Asked Questions

How long does tennis elbow take to heal with physiotherapy?+

Typically 6–12 weeks with physiotherapy. Chronic cases lasting over 6 months may take 3–6 months of treatment. The key is eccentric strengthening combined with manual therapy.

Should I rest completely for tennis elbow?+

No, complete rest is counterproductive for tendinopathy. Graded loading through eccentric exercises is the evidence-based treatment that promotes tendon remodelling.

Is tennis elbow physiotherapy available in Ahmedabad?+

Yes. Dr. Maitri Kothari at PhysioBuddy Chandkheda treats tennis elbow with manual therapy and eccentric loading protocols.

Can I still use my arm during tennis elbow treatment?+

Yes, with modifications. Dr. Maitri Kothari will advise on which activities to modify, how to grip tools differently, and what activities to avoid temporarily.
