Two jobs at once: protect and rebuild
A hip replacement swaps a worn, painful joint for a smooth new one, and most patients are standing within a day of surgery. But the muscles and capsule around the new hip need weeks to heal and strengthen, and until they do, certain movements can push the ball out of its socket. Good rehab therefore runs on two tracks at the same time: strict movement precautions in the early weeks, and steady, progressive strengthening so you can eventually forget you have an implant at all.
This programme sits within our post-surgical rehabilitation service. Dr. Maitri Kothari (BPT, MPT in Musculoskeletal and Sports) has more than 10 years of post-operative rehab experience, and the early weeks are delivered through home visits across Chandkheda, Motera, Gandhinagar and nearby North Ahmedabad, because that is where the real challenges live: your bed height, your bathroom, your stairs.
The precautions that keep your hip safe
Exact precautions depend on the surgical approach; the common posterior approach usually means, for roughly the first 6 to 12 weeks:
- Do not bend the hip beyond 90 degrees, so no low sofas, low stools or squatting toilets
- Do not cross the operated leg over the midline of your body, sitting or lying
- Do not twist the operated leg inward (toes rotating in) while standing or turning
- Use a raised chair and a commode raiser; sleep on your back with a pillow between the knees as advised
- Turn your whole body instead of pivoting on the operated leg
Your surgeon's instructions win. Approaches differ, and some modern techniques carry fewer restrictions. We read your discharge summary, speak with your surgeon's team where needed, and train you and your family in the exact precautions that apply to your hip, until safe movement becomes automatic.
Weeks 1 to 2: safe foundations
The first goals are calm swelling, a healing wound, and confident basic movement inside the precautions:
- Ankle pumps hourly, plus breathing exercises, to support circulation after surgery
- Gluteal and quadriceps activation exercises in lying
- Heel slides and supported hip movements within the safe range
- Bed mobility and transfers done the precaution-safe way, taught to you and one family member
- Walker-assisted walking for short, frequent indoor distances
Weeks 3 to 6: strength and independent walking
As healing progresses, the emphasis shifts to the muscles that stabilise your pelvis, especially the hip abductors on the outside of the hip. Weakness here causes the classic lurching limp, so we target it early with graded exercise therapy: standing hip abduction, mini squats to safe depth, step-ups on a low step, and bridge progressions. Walking progresses from walker to stick, posture and step length are corrected before habits form, and stair technique is practised. If exercise soreness slows you down, modalities such as IFT or TENS can keep you comfortable enough to train consistently.
Weeks 7 to 12: back to your life
Most patients now walk indoors without support and outdoors with growing confidence. We work on balance reactions, longer walking distances, car entry and exit, and the specific movements your routine demands, standing kitchen work, society walks, temple visits, office days in Gandhinagar or GIFT City. Around this stage your surgeon typically reviews you and relaxes precautions; we then retrain the movements that were off-limits, like comfortable sitting on lower seats, in a controlled way.
What we watch for throughout
At every session we screen for warning signs: sudden severe hip pain with a sensation of the joint slipping, a leg that suddenly looks shorter or rotated, calf pain or swelling, fever, or wound redness and discharge. Any of these means your surgeon is the next call, not another exercise. This vigilance is part of why supervised rehab beats a photocopy of exercises handed over at discharge.
Why home rehab works so well for hips
Hip precautions are about furniture as much as anatomy. At a home visit we can raise your chair with a cushion, check your bed height, plan the safest bathroom route and rearrange the one rug that is waiting to cause a fall. Families tell us this practical safety audit alone was worth the visit. Once you are steady, sessions can continue at our Chandkheda clinic, behind Satyamev Hospital, if you prefer equipment-based strength work. You can read recovery experiences from our post-surgical patients on the reviews page.

Frequently Asked Questions
How soon after hip replacement does physiotherapy begin?
Standing and a few steps usually happen within a day of surgery, guided by the hospital team. Structured rehab continues from the day you reach home. We typically make the first home visit within a day or two of discharge, so no momentum is lost.
How long do hip precautions last?
Commonly 6 to 12 weeks, but it depends on the surgical approach and your surgeon's protocol; some newer approaches have minimal restrictions. We follow your surgeon's specific instructions and train you until precaution-safe movement is second nature.
When can I walk without a walker or stick?
Most patients move from walker to stick between weeks 2 and 4, and walk unaided indoors by weeks 4 to 8. We progress you based on strength and balance rather than the calendar, because walking aids removed too early lead to limps that are hard to unlearn later.
Can I sit on the floor or use an Indian toilet after hip replacement?
Not during the precaution period, and for many patients floor sitting and deep squatting remain restricted long term, depending on the implant and approach. This is a decision for your surgeon; whatever they permit, we train the safest technique for it. A commode raiser is essential in the early months.
Do you provide hip replacement physiotherapy at home in Ahmedabad?
Yes, home visits are our standard for the early phase, covering Chandkheda, Motera, Gandhinagar and nearby North Ahmedabad. Home rehab also lets us make your furniture and bathroom precaution-safe, which a clinic session cannot do.
How many physiotherapy sessions are needed after hip replacement?
A typical course runs about 8 to 12 weeks, starting at 2 to 4 sessions a week and tapering as you become independent. Ahmedabad clinic sessions generally range from Rs 300 to 800 and home visits cost somewhat more; we confirm an exact quote on call or WhatsApp at +91 78619 29326.
