What is TENS?
TENS stands for transcutaneous electrical nerve stimulation, which is a long name for a simple idea: a small device passes a mild, adjustable electrical current through pads stuck on the skin near the painful area. The current stimulates the touch nerves under the pads, and those signals reach the spinal cord faster than pain signals do, effectively closing the gate on pain before it registers fully in the brain. With certain settings, TENS also nudges the body to release its own natural pain-relieving chemicals, endorphins.
You have used this principle all your life without a machine: when you bang your shin and rub it, the rubbing sensation dampens the pain. TENS does the same job more precisely and for longer.
What a session feels like
Two or four pads are placed on or around the painful area, and the intensity is raised gradually until you feel a distinct but comfortable tingling or buzzing. It should feel strong enough to notice, never sharp or burning. A clinic session usually runs 15 to 20 minutes, during which many patients feel the pain fade into the background. Relief often continues for a period after the machine is switched off, and that pain-free window is valuable: it is the best time to do the exercises that produce lasting change.
What the evidence says, honestly
Research on TENS is mixed, partly because settings and pad placements vary so widely between studies. The fair summary: many people get genuine short-term pain relief, some feel little, and TENS by itself does not fix the underlying problem. That is why we never build a treatment plan on TENS alone. Safety, and who should avoid TENS
TENS is one of the safest modalities we use, but it is avoided in people with pacemakers or implanted electronic devices, over the abdomen or lower back during pregnancy, on the front of the neck, over broken or infected skin, and in areas with reduced sensation. Does TENS therapy hurt?
No. You feel a clear tingling or buzzing that stays within your comfort level, and you help set that level. If anything ever feels sharp or burning, the settings are adjusted immediately. Most patients find sessions comfortable, even pleasant.
How long does the pain relief last after a TENS session?
It varies from person to person, from relief mainly during the session to several hours afterwards. We treat that window as an opportunity: exercises done while pain is dialled down build the strength and mobility that produce lasting improvement.
Is TENS a permanent cure for pain?
No, and we will be straight with you about that. TENS manages the pain signal; it does not repair the underlying problem. Lasting change comes from the exercise, hands-on treatment and habit changes that TENS makes more comfortable to perform.
Should I buy a TENS machine for home use?
Sometimes it is worth it, particularly for long-standing pain that flares between sessions. But placement and settings matter more than the gadget, and using it as a reason to rest completely backfires. If you have or want one, ask us; we will teach you correct use as part of your plan.
Who should not use TENS?
Anyone with a pacemaker or implanted electronic device, pregnant women over the abdomen or lower back, and anyone with broken skin, infection or numb areas at the pad sites. The front of the neck is also off-limits. We screen for all of this before using it.
What is the difference between TENS and IFT?
Both use electrical currents for pain relief. TENS works closer to the surface through two pads and is simple and portable; IFT uses four pads with crossing currents that reach deeper tissue more comfortably and is a clinic treatment. We choose based on how deep the problem sits and how you respond.
