Post-Surgery Rehab Programme

Physiotherapy After Knee Replacement Surgery

The surgeon gives you a new knee; physiotherapy is what teaches you to use it. Here is the week-by-week road from hospital bed to comfortable walking, and how we support it at home after discharge.

Why rehab decides the result of your knee replacement

Total knee replacement is one of the most successful surgeries in orthopaedics, but the implant is only half the outcome. The other half is rehabilitation. A knee that is not moved early and progressively stiffens, and a stiff replaced knee is much harder to loosen later than it is to keep mobile from day one. That is why your surgeon will insist physiotherapy starts within a day of surgery, and why the 12 weeks after the operation matter so much.

At Physio Buddy, knee replacement rehab is one of our core post-surgical rehabilitation programmes. Dr. Maitri Kothari (BPT, MPT in Musculoskeletal and Sports) has guided these recoveries for over a decade, and because most patients cannot travel comfortably in the early weeks, we run the first phase through home visits across Chandkheda, Motera, Gandhinagar and nearby North Ahmedabad.

The road map at a glance

PhaseWeeksMain goals
Early protection1 to 2Control swelling, wake up the quadriceps, bend to about 90 degrees, walk safely with a walker
Building motion and strength3 to 6Bend towards 110 to 120 degrees, straighten fully, walk longer with less support, manage stairs
Return to life7 to 12Strength, balance and confidence for daily routines, walks, and light activity

Weeks 1 to 2: protect, move, activate

This phase usually starts in hospital and continues at home after discharge. The enemies are swelling, pain-related guarding, and a quadriceps muscle that switches off after surgery. Typical work includes:

Getting the knee fully straight is as important as bending it; we check extension at every visit, because a knee that loses full straightening walks with a limp for years.

Weeks 3 to 6: the window that decides your bend

Scar tissue matures in this period, so the range you build now is largely the range you keep. Sessions progress to:

Weeks 7 to 12: return to your routine

Now the knee stops being the centre of your day. We push strength and balance so you can stand in the kitchen through a full meal preparation, walk your society compound comfortably, manage stairs confidently, and get in and out of an auto without planning it like an expedition. Patients who were active before surgery progress towards longer walks and gentle cycling. Kneeling and squatting depend on your surgeon's advice and implant type, so we align the plan with their guidance.

How Physio Buddy runs this programme

We assess range, strength, swelling and walking pattern at every session and progress the plan against milestones rather than a fixed script, because a 55-year-old and an 80-year-old do not recover on the same clock. Early weeks happen at your home; once you are moving well, you can continue at our Chandkheda clinic near Satyamev Hospital if you prefer supervised equipment work. Progress updates go to your family, and to your surgeon when requested. You can read what recovering patients say, including Samrat's account of his knee replacement recovery, on our reviews page.

Call your surgeon promptly if you notice increasing redness, warmth or discharge at the wound, fever, calf pain or swelling, chest pain or breathlessness, or a sudden inability to bear weight. These are medical matters first; rehab resumes once your doctor clears it.

What recovery honestly feels like

Expect good days and cranky days. Swelling that fluctuates for two to three months is normal, night aching in the early weeks is common, and clicking sounds from the implant are usually harmless. What should trend steadily is function: each week you should walk a little farther, bend a little more, and need a little less support. If that trend stalls for more than a couple of weeks, we reassess rather than pushing the same plan harder.

Frequently Asked Questions

When should physiotherapy start after knee replacement?

Usually within 24 hours of surgery, while you are still in hospital. The hospital team handles the first day or two, and we take over from discharge, typically with home visits in the first weeks. Starting early is the single best protection against a stiff knee.

How long does full recovery take?

Most people walk independently indoors by 3 to 6 weeks and feel largely back to daily life by 3 months. Strength and stamina keep improving for 6 to 12 months. Your age, pre-surgery fitness and consistency with exercises all shift the timeline, so we set personal milestones rather than promising fixed dates.

Do you do home visits after knee replacement in Ahmedabad?

Yes, home-based rehab after discharge is exactly how we run the early phase. We cover Chandkheda, Motera, Gandhinagar and nearby North Ahmedabad areas. Once you are mobile, you can continue at home or shift to our Chandkheda clinic, whichever suits you.

How much knee bend should I expect?

Most patients reach about 90 degrees by 2 weeks and 110 to 120 degrees by 6 to 12 weeks, which covers chairs, stairs, car seats and most of daily life. Deep squatting and full kneeling depend on the implant and your surgeon's advice. The critical work happens in weeks 3 to 6, before scar tissue fully matures.

Is the exercise supposed to hurt?

Some stretching discomfort during bending work is normal and expected; sharp pain, or pain plus swelling that worsens after every session, is not. We work at the edge of discomfort, not through pain, and adjust the load based on how your knee responds within 24 hours of each session.

How many sessions will I need in total?

A typical course runs 2 to 5 sessions a week early on, tapering as you become independent with your exercises, over roughly 8 to 12 weeks. The exact number depends on your progress. Ahmedabad clinic sessions generally range from Rs 300 to 800, home visits cost somewhat more, and we give an exact quote on call or WhatsApp.

What happens if I skip physiotherapy after surgery?

The most common outcomes are a stiff knee that will not bend enough for chairs and stairs, a lingering limp from incomplete straightening, and weakness that keeps you dependent on a stick. Stiffness that sets in is far harder to treat later, sometimes needing manipulation under anaesthesia, so consistent early rehab is genuinely the cheaper path.
