What is therapeutic ultrasound?
Therapeutic ultrasound is a physiotherapy modality that sends high-frequency sound waves, far above what the ear can hear, into injured tissue through a small handheld probe. This is not the imaging ultrasound used for scans; instead of taking pictures, the sound energy is absorbed by tissues like tendon, ligament and muscle, where it produces gentle warming deep below the surface and a micro-massage effect at the cellular level. The aim is to ease pain, soften tight or scarred tissue, and encourage local blood flow in the healing area.
What a session feels like
Honestly, not much, and that surprises people. The therapist applies a water-based gel over the injured area, then glides the probe in slow circles for around 5 to 10 minutes. You may feel light pressure and sometimes mild warmth; many patients feel nothing at all, which does not mean nothing is happening, the sound waves work below the level of sensation. There are no needles, no electric sensation and no downtime. It is common to feel gradual easing of pain and stiffness over the following sessions rather than a dramatic change on the table.
What the evidence says, in plain language
We believe in telling patients the truth about modalities. Research on therapeutic ultrasound shows modest benefits for some soft tissue conditions and less convincing results for others, and no serious clinician treats it as a stand-alone cure. Used well, it is a supporting act: it can calm pain and prepare tissue so that the treatments with the strongest evidence, exercise therapy and manual therapy, can do their work sooner and more comfortably. Does ultrasound therapy hurt?
No. Most people feel only the glide of the probe and the cool gel, sometimes a mild warmth. If you ever feel sharp heat or discomfort during treatment, tell the therapist immediately so settings can be adjusted, but that is rare in practice.
How many ultrasound sessions will I need?
It depends on the condition, but ultrasound is usually applied over a course of several sessions alongside exercise and hands-on treatment, then tapered as the tissue improves. We reassess regularly, and if it is not adding value to your case we stop it rather than continuing by habit.
Is this the same ultrasound used for pregnancy scans?
Same physics, completely different job. Imaging ultrasound uses sound waves to create pictures; therapeutic ultrasound delivers sound energy into tissue to ease pain and support healing. No image is produced during treatment.
Can ultrasound alone cure my tendon pain?
No single machine cures tendon pain, and we will not pretend otherwise. Tendons recover through progressive loading, meaning graded exercise, and ultrasound can make that process more comfortable and help settle symptoms. Think of it as a supporting treatment, not the main one.
Who should not have ultrasound therapy?
